A current dog owner needs to complete this form when their dog has moved to a new home.
Section 48 of the Dog Control Act 1996 states that where the ownership of any dog is changed, the previous owner and the new owner shall each within 14 days give written notice to the territorial authority or territorial authorities concerned of the change of ownership and of the residential address of the new owner and the address at which the dog will ordinarily be kept.
Notification of change of ownership for a dog must be by consent of both parties. Animal Management reserves the right to correct unauthorised changes that have been submitted.
When transferring more than one dog to a new owner or owners, please complete a separate form for each dog being transferred.
It is an offence to make a false statement relating to dog registration.
