Complete this form when your dog has moved to a new home.

Section 48 of the Dog Control Act 1996 states that where the ownership of any dog is changed, the previous owner and the new owner should each, within 14 days, give written notice to the territorial authority or territorial authorities concerned, of the change of ownership and of the residential address of the new owner and the address at which the dog will ordinarily be kept.

Important

  • Notification of a change of ownership for a dog must be made with the consent of both parties. Providing false or misleading information is a breach of the Dog Control Act 1996.
  • We reserve the right to correct unauthorised or incorrect changes that have been submitted. 
  • When transferring more than one dog to a new owner, please complete a separate form for each dog being transferred.
